At Work:
Tell your supervisor immediately – don’t wait, no matter how minor you think your injury is. Fill out a First Report of Injury which should be available right there at work, get the names and phone numbers of all persons who witnessed the injury. Remember, if you get hurt at work, nobody needs to be at fault in order to get Workers’ Compensation. Simply stated, you only need to show that you hurt yourself while working.
In a Car:
If you are severely hurt in a car accident, wait for help to arrive. Make sure the police are contacted. Get the name, address, license and insurance information from the other party. Get names and phone numbers of any witnesses if possible. Contact your insurance carrier as soon as possible.
At a Retail Store:
Inform the store manager or other store employee immediately. If you need assistance for your injuries, make sure the store contacts an ambulance for you. Get the names and addresses of any potential witnesses who saw the accident.
